29 CFR 1926.1431(j)(3): After proof testing, a competent person must inspect the platform and rigging to determine if the test has been passed. If any deficiencies are found that pose a safety hazard, the platform and rigging must not be used to hoist personnel unless the deficiencies are corrected, the test is repeated, and a competent person determines that the test has been passed. (See § 1926.1417 for tag-out and related requirements.) Barge 2- On or about, June 2, 2026, and at times prior thereto, employees engaged in construction activities were exposed to injury because the rigging used for the platform was not inspected and had kinks in the four leg bridle sling. Excessive rust was also observed.

29 CFR 1926.251(e)(8): Synthetic web slings were not immediately removed from service if any of the following conditions specified in 1926.251(e)(8)(i) through (e)(8)(v) were present: (i) Acid or caustic burns; (ii) Melting or charring of any part of the sling surface; (iii) snags, punctures, tears, or cuts; (iv) broken or worn stitches; or (v) distortion of fittings. East side floating section - On or about, June 2, 2026, and at times prior thereto, employees performing lifting operations, were exposed to injury in that synthetic web slings were observed with visible damage including tears and punctures along the body of the sling.
