29 CFR  1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components are removed from service.  Employees were exposed to fall hazards. Employees engaged in warehouse picking activities using the Talon self-retracting lifeline, SN: 2036, were exposed to fall hazards in that the employer did not remove the damaged personal fall protection system from service. Employees engaged in picking activities are required to utilize self-retracting lifelines to prevent falls.
