29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel:                                        On or about February 27, 2021, at the Lion Oil Refinery in El Dorado, AR workers were exposed to process hazards when the host employer did not implement safe work practices to prohibit the use of heavy equipment in a roadway with temporary naphtha piping.

29 CFR 1910.119(l)(1):The employer did not implement procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process:  On or about February 27, 2021 and at times prior thereto in at the Lion Oil Refinery in El Dorado, AR the employer did not implement procedures to manage changes to process unit.  (a) Piping was added to the process in Unit 6 to allow the Stripper Column to be charged with Naphtha from Unit 9.  (b) Piping was added to the process in Unit 9 to allow the Unit 6 Stripper Column to be charged with Naphtha from the Unit 9 process.  (c) Process piping was routed in the roadway between Unit 6 and Unit 9.

29 CFR  1910.119(l)(4): Process safety information was not updated when a process change covered by 29 CFR 1910.119(l) occurred:   On or about February 27, 2021 workers in the Lion Oil Refinery in El Dorado, AR,  were exposed to hazards that impact safety when a change to a process covered by 29 CFR 1910.119(l) was made without updating process safety information.  (a) In Unit 6, piping was added to the bottom of the Separator Column, but the process safety information, including but not limited to Process Flow Diagrams, Piping and Instrumentation Diagrams, and Piping Specifications were not updated.  (b) In Unit 9, a change to Naphtha Piping was made to allow for connection directly to the Unit 6 Separator Column but the process safety information, including but not limited to Process Flow Diagrams, Piping and Instrumentation Diagrams, and Piping Specifications were not updated.

29 CFR  1910.106(c)(3): Above ground piping with threaded joints used for transfer of flammable liquid was not secured to prevent disengagement at the fitting and the piping system was not so designed that any spill resulting from such disengagement could not unduly expose persons, important buildings or structures, and could be readily controlled by remote valves:   (a) On or about February 27, 2021 in the process area near the Stripper Column in Unit #6 at the Lion Oil Refinery in El Dorado, AR, workers were exposed to fire and explosion hazard when above ground threaded piping used for transferring Naphtha between Units #6 and #9 was not secured to prevent disengagement of threaded joints at the fittings and/or so designed that any spill resulting from such disengagement could not unduly expose persons, important buildings or structures. Additionally, the piping was not readily controlled by remote valves.

29 CFR 1910.106(c)(7): Before being covered, enclosed, or placed in use, all piping was not hydrostatically tested to 150 percent of the maximum anticipated pressure of the system, or pneumatically tested to 110 percent of the maximum anticipated pressure of the system, but not less than 5 pounds per square inch gauge at the highest point of the system. Test was not maintained for a sufficient time to complete visual inspection of all joints and connections, but for at least 10 minutes:  (a) On or about February 27, 2021 in the process area around the Stripper Column in Unit 6 at the Lion Oil Refinery in El Dorado, AR, workers were exposed to fire and explosion hazard when piping installed to charge the Unit 6 Stripper column with Naphtha from Unit 9 was not hydrostatically tested prior to being placed in use.
