29 CFR  1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:   a) Frank Yavorsky at the address of 945 South Main Street in Bowling Green, Ohio: On or about March 1, 2021 the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)Frank Yavorsky at the address of 945 South Main Street in Bowling Green, Ohio:  On or about March 1, 2021 the employer did not ensure that employees had been trained in the appropriate fall protection such as, but not limited to the use of personal fall arrest systems or alternative measures. Employees were exposed to hazards associated with falls of approximately nine feet.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.:  a) Frank Yavorsky at the address of 945 South Main Street in Bowling Green, Ohio:  On or about March 1, 2021 the employer did not ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. Employees tearing off roofing material on the structure were exposed to falls of approximately nine feet to the ground below.
