29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:   a) 138 Palmer Ave., West Springfield, MA. On or about March 2, 2021 employees were exposed to a 6'9" fall hazard due to a missing guard rail on the walking working surface of the main sorting machine in the southwest side of the recycling building.  b) 138 Palmer Ave., West Springfield, MA. On or about March 2, 2021 employees were exposed to a 12'5" fall hazard due to a improperly guarded  sorting chute  at the bottom of ladder on the walking working surface of the main sorting machine in the east side of the recycling building.

The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) 138 Palmer Ave., West Springfield, MA : On or about March 2, 2021 and at times prior, The employer had not established a program consisting of written energy control procedures, employee training and periodic inspections for those employees that operate and perform service and maintence activities on the recycling sorting machines in the main building where the unexpected energizing, startup or release of stored energy could occur and cause injury. Contractors only turned off machines for maintenance and employees did not use locks or any other procedures when they unclogged conveyors in the recycling sorting machine exposing themselves to amputation, struck by and caught in hazards.
