29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were not protected from crushed by hazards by not having operator restraint devices installed on powered industrial equipment:  a. 55 Wester Ave., Metuchen, NJ - Shop and loading docks  The employer did not ensure that powered industrial equipment used in the movement of materials within the shop and on the loading docks were equipped with operator restraint devices.  Employees engaged in the manufacture of graphics printing machines operated forklifts that included, but were not limited to, Yale, TCM, and Caterpillar forklifts that did not have seatbelts installed that were  intended to assist the operator in reducing the risk of entrapment of the operator's head and/or torso between the truck and ground in the event of a tip over.  Violation observed on or about August 20, 2021

29 CFR  1910.110(f)(6)(ii): Containers were not stored in a suitable enclosure or otherwise protected against tampering.  a. 55 Wester Ave., Metuchen, NJ - Warehouse docks  The employer did not ensure that propane fuel containers were stored in a suitable enclosure to prevent tampering.  Employees who operated propane fueled powered industrial trucks, stored propane bottles on a racking system outside of the building that did not enclose the bottles or protect from tampering. 29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. 55 Wester Avenue, Metuchen, NJ - Production Area  The employer did not ensure that machines that create hazards created by point of operation and rotating parts were guarded in a manner to prevent those hazards. An employee engaged in milling operations, operating a Kuraki, KBT-11DX Milling Machine, was able to step into the area of operation and became caught in rotating parts of the machine.  Violation occurred on or about March 2, 2021  b. 55 Wester Avenue, Metuchen, NJ - Production Area   The employer did not ensure that machines that create hazards created by point of operation and rotating parts were guarded in a manner to prevent those hazards. An employee engaged in milling operations, operating a Herbert-DeVlieg, Spriamatic Jigmill, was able to step into the area of operation of the rotating parts.  Violation observed on or about March 2, 2021  c. 55 Wester Avenue, Metuchen, NJ - Production Area  The employer did not ensure that machines that create hazards created by point of operation and rotating parts were guarded in a manner to prevent those hazards. An employee engaged in milling operations, operating a Shibaura, MPE-2650 milling machine, was able to step into the area of operation of the rotating parts.  Violation observed on or about March 2, 2021  d. 55 Wester Avenue, Metuchen, NJ - Production Area  The employer did not ensure that machines that create hazards created by point of operation and rotating parts were guarded in a manner to prevent those hazards. Employees engaged in machine shop activities used drill presses that included, but was not limited to, a Clausing drill press and a Quad Machinery, Powermatic, Model 1150 drill press, that did not have spindle guards to protect against being caught in rotating machine parts.   Violation observed on or about March 2, 2021

Employees engaged in the manufacture of metal graphics equipment, used chemicals that included, but was not limited to propane, acetylene, oxygen, argon, Trim MicroSol 685 lubricant, and Soltrol 130, were not provided training on the hazards associated with the chemicals being used.   Violation occurred on or about March 2, 2021
