29 CFR  1926.451(b)(2)(i):Each ladder jack scaffold, top plate bracket scaffold, roof bracket scaffold, and pump jack scaffold was not at least 12 inches (30 cm) wide:  a. Tom Keene at the worksite located at Winesap Trail (Lot 36) in Avon, Ohio: On or about March 3, 2021, the employer did not ensure each scaffold plank was at least 12-inches wide.  Employees were exposed to an approximate 16.5-ft fall hazard in that a single 2"x8" board was used as a platform between a ladder jack support and pump jack scaffold.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1926.451(b)(5)(ii):The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.  a. Tom Keene at the worksite located at Winesap Trail (Lot 36) in Avon, Ohio:  On or about March 3, 2021, scaffold planks were extended approximately three to five feet past support brackets, exposing employees to falls of approximately 16.5 ft. while performing siding work.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Tom Keene at the worksite located at Winesap Trail (Lot 36) in Avon, Ohio: On or about March 3, 2021, personal fall arrest systems or guardrail systems were not in place at each end of the ladder/pump jack scaffold system to protect employees from an approximate 16.5-foot fall hazard while performing siding work.  b. Tom Keene at the worksite located at Winesap Trail (Lot 36) in Avon, Ohio: On or about March 3, 2021, personal fall arrest systems or guardrail systems were not in place to protect employees from an approximate 16.5-ft fall hazard while performing siding work in that the pump jack scaffold system was not equipped with a mid-rail between the work bench and walking surface.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. Tom Keene at the worksite located at Winesap Trail (Lot 36) in Avon, Ohio: On or about March 3, 2021, an employee was exposed to a 13-ft fall hazard in that an extension ladder, used to access a porch overhang, was used without extending the ladder's side rails at least three feet above the landing surface.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
