29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about September 21, 2021, employees were exposed to a fall of approximately 16 feet while performing siding activities on a residential home located at 36584 Capri Lane, North Ridgeville, Ohio 44039. An employee was working from a ladder pick scaffold and the employer did not ensure that he was using fall protection. Tom Keene DBA T'S Exterior LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1926.451 (g)(1)(v)(ii), which was contained in OSHA inspection number 1517593, citation number 1, item number 1c and was affirmed as a final order on (5/21/21), with respect to a workplace located on Lot 36 Winsap, Avon, Ohio 44011.
