MDM & Sons, LLC, on or about March 2, 2021 at the address of 519 Canterbury Blvd. Perrysburg, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as preventing falls from heights on the worksite.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) MDM & Sons, LLC, on or about March 2, 2021 at the address of 519 Canterbury Blvd. Perrysburg OH: The employer did not ensure employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing various roofing operations, including but not limited to laying sheeting and installing trusses. Employees were unable to identify fall hazards on the job site, exposing them to fall hazards of approximately 18ft.

Employees were exposed to the hazards of eye injury from flying objects.
