On or about March, 3, 2021, the employer did not ensure that guardrails were installed a long all open sides of the work platform. This exposed the employees working from the work platform to a fall hazard.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: On or about March, 3, 2021, the employer was in use of an unauthorized work platform, that was attached to the forks of a rough terrain powered industrial truck (PIT), without the written approval from the manufacturer. The work platform did not have guardrails installed along all sides and extend more than 10 inches beyond the wheelbase of the PIT exposing the employees working from the work platform to a fall hazard.
