29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Oscar Torres located at 60 Nadine Place WESTERVILLE, OH 43081: On and preceding March 10, 2021, employees were performing roof work on a two story residential home at heights up to 20 feet without fall protection, PPE and improperly configured ladders. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

The following condition exposed employees to struck-by hazards to the eyes.

Roofers were installing shingles and other roofing materials at heights up to 20 feet without fall protection. The following condition exposed them to falls up to 20 feet.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a) Oscar Torres located at 60 Nadine Place WESTERVILLE, OH 43081: On and preceding March 10, 2021, the employer did not ensure that the ladder used for access to the roof extended at least three feet beyond the surface they were accessing. Roofers were using an extension ladder to access the roof of a two story residential home with the ladder extended 6 inches beyond the eave and was not secured to the house. The following condition exposed them to falls up to 12 feet
