29 CFR 1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: a) Oscar Elias Torres located at 2270 Pinebrook Rd Upper Arlington, OH 43220: On and preceding December 2, 2021, the employer did not initiate and maintain safety and health programs for employees removing roof shingles, thereby exposing employees to the hazards associated with performing roofing work. Oscar Elias Torres was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1518667, citation number 1, item number 1 and was affirmed as a final order on April 15, 2021, with respect to a workplace located at 60 Nadine Place, WESTERVILLE, OH 43081.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Oscar Elias Torres located at 2270 Pinebrook Rd Upper Arlington, OH 43220: On and preceding December 2, 2021, the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Roofers were removing shingles and other roofing materials at heights up to 20 feet without fall protection. The following condition exposed them to falls up to 20 feet. Oscar Elias Torres was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1518667, citation number 1, item number 4 and was affirmed as a final order on April 15, 2021, with respect to a workplace located at 60 Nadine Place, WESTERVILLE, OH 43081.
