29 CFR 1926.651(b)(3):When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a- 32 Milford Lane/Somerset Drive, Suffern NY: Employees engaged in gas line installation ruptured a gas line when excavating with a Komatsu back hoe as hand digging was not utilized to locate an underground gas line . Condition noted on or about 03/09/2021
