29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: At a jobsite located at 1055 Waters Edge Derby, Kansas employees engaged in residential framing activities were exposed to electrical shock hazards in that the extension cord used to supply power to the Milwaukee reciprocating saw was damaged in two locations.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. At a jobsite located at 1055 Waters Edge Derby, Kansas employees engaged in residential framing activities were exposed to fall hazards and crushing hazards. Employees were exposed in that the scaffold platform was not attached to the forks or forklift.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At jobsite located at 1055 Waters Edge Derby Kansas, employees engaged in residential framing activities were exposed to fall hazards of approximately 10 feet from the roof to the ground in that fall protection was not utilized. The Eugene Hickey dba Gene Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1504946, citation number 1, item number 2 and was affirmed as a final order on 02/02/21, with respect to a workplace located at 1031 Waters Edge Derby Kansas.
