29 CFR 1910.147(d)(2):  Machine or equipment shutdown. The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.  On or about 3/09/2021, Apex Metal Finishing Company had not ensured equipment was locked out prior to performing maintenance.  An employee suffered an amputation while attempting to service the #8 barrel tumbler.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed.  On or about 3/09/2021, Apex Metal Finishing Company not conduct periodic inspections on their authorized employees or for several machines in the plant to include, but not limited to the barrel tumblers.

29 CFR  1910.212(a)(3)(ii):  The point of operation of machines, whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, was so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about 3/09/2021, employees at Apex Metal Finishing Company were exposed to point of operation hazards.  A Westward 1KEN7 Floor Drill Press was missing a chuck guard.
