29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and were not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Pit below Press #168. On or about February 28, 2021, employees performing servicing and maintenance including, but not limited to, inspection, trouble-shooting, and die setting, did not follow the energy control (lockout/tagout) procedures before attempting to trouble-shoot the GPI 60-ton mechanical power press. A die-setting employee received a fingertip amputation when the press activated while his hand was near moving parts of the machine. b.) NST Falls Creek, LLC - Die Setting Operations: On or about February 28, 2021 and at times prior there to: Personnel were not performing LOTO when performing PM and Sizing Press set-up work.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by nip points and moving parts: a) Pit below Press #168. On or about February 28, 2021, the core rod associated with the GPI 60-ton mechanical power press was not guarded to prevent employees from having hands near moving parts. A die-setting employee received a fingertip amputation when the press activated while his hand was in the moving parts of the core rod of the machine.
