A die-setting employee received a fingertip amputation when the press activated while his hand was near moving parts of the machine.  b.) NST Falls Creek, LLC - Die Setting Operations: On or about February 28, 2021 and at times prior there to:  Personnel were not performing LOTO when performing PM and Sizing Press set-up work.
