29 CFR 1904.31(b)(2): The employer did not record injuries and illnesses for employees obtained from a temporary help service, employee leasing service, or personnel supply service, and were supervised by the employer on a day-to-day basis: Hodge Company, 3145 5th Ave, East Moline, IL, USA: On or about March 12, 2021, the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for the calendar year 2021. a) On or about February 5, 2021, a recordable illness occurred when an employee placed by DES Employment Group, received a musculoskeletal disorder of the left hand resulting in both lost time and restricted work activity. b) On or about March 6, 2021, a recordable injury occurred when an employee placed by DES Employment Group, received an injury to the left knee resulting in lost work time and restricted work activity.
