29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed by, caught in, and fall hazards when loading, operating, and unloading three company-made dumbwaiter machines.   Location: 75 Logan St, Gardner, MA On March 10, 2021, and at times prior, employees were exposed to crushed by, caught in, and fall hazards when operating three company-made dumbwaiters to move materials between production floors.  The loading compartment had no device to adequately lock it into position when door was open and being loaded.  The doors of the stationary hoist-operated dumbwaiters were able to be opened when the loading compartment was at any point in the vertical shaft.
