On or about March 17, 2021, at the above addressed jobsite, two portable ladders used to access the roof from the front of the house were extended approximately two (2) feet above the upper landing surface, thereby exposing employees to fall hazards. b. On or about March 17, 2021, at the above addressed jobsite, a portable ladder used to access the roof from the back of the house was extended approximately one (1) foot six (6) inches above the upper landing surface, thereby exposing employees to fall hazards.

Luis Gutierrez dba Luis Gutierrez was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1268195, citation number 1, item number 1-a, and was affirmed as a final order on March 27, 2018, with respect to a workplace located at 17864 Martie Rd. in Mokena, Illinois.
