On or about March 17, 2021, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing the employees to hazards associated with eye injuries.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a.  On or about March 17, 2021, at the above addressed jobsite, two portable ladders used to access the roof from the front of the house were extended approximately two (2) feet above the upper landing surface, thereby exposing employees to fall hazards.  b.  On or about March 17, 2021, at the above addressed jobsite, a portable ladder used to access the roof from the back of the house was extended approximately one (1) foot six (6) inches above the upper landing surface, thereby exposing employees to fall hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about March 17, 2021, at the above addressed jobsite, employees performing residential roofing operations, eleven (11) feet eight (8) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about March 17, 2021, at the above addressed jobsite, employees performing residential roofing operations, ten (10) feet seven (7) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  Luis Gutierrez dba Luis Gutierrez was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1268195, citation number  1, item number 1-a, and was affirmed as a final order on March 27, 2018, with respect to a workplace located at 17864 Martie Rd. in Mokena, Illinois.
