29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing operations.  This was most recently observed on March 19, 2021, at 2115 209th St, Elkhorn, Nebraska when employees were installing shingles to a residential structure and they were 13 feet above the ground with no fall protection in use.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using portable ladders during roofing operations.  This was most recently observed on March 19, 2021, at 21115 209th St, Elkhorn, Nebraska, when workers used a portable ladder with side rails that did not extend at least 3 feet past the upper landing area.
