29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) A & H Roofing Services, LLC located at 467 Landings Loop, Westerville, OH 43082: On and preceding March 23, 2021, employees were installing shingles on a re-roof project of on a two story residential home at heights up to 27 feet without fall protection or PPE. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted.  A & H Roofing Services, LLC  was previously cited for a violation of this occupational safety and health standard its equivalent standard 29 CFR 1926.20(b)(1), while performing work under the owner's name, Abel Hernandez, which was contained in OSHA inspection number 1458624, citation number 1, item number 1 and was affirmed as a final order on March 31, 2020, with respect to a workplace located at 6575 Greenway Loop, Dublin, OH 43016.

29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.   a) A & H Roofing Services, LLC located at 467 Landings Loop, Westerville, OH 43082: On and preceding March 23, 2021, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were installing shingles with pneumatic nail guns on a roof that varied from 10-27 feet. The employees were not utilizing eye or fall protection and using improperly configured ladders which exposed them to falls up to 27 feet and struck by hazards to the eyes.  A & H Roofing Services, LLC  was previously cited for a violation of this occupational safety and health standard its equivalent standard 29 CFR 1926.21(b)(2), while performing work under the owner's name, Abel Hernandez, which was contained in OSHA inspection number 1458624, citation number 1, item number 2 and was affirmed as a final order on March 31, 2020, with respect to a workplace located at 6575 Greenway Loop, Dublin, OH 43016.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   a) A & H Roofing Services, LLC located at 467 Landings Loop, Westerville, OH 43082: On and preceding March 23, 2021, the employer did not ensure that employees wore eye protection while utilizing pneumatic nail guns. Workers were installing roofing shingles using pneumatic nail guns powered at 120 psi without the use of eye protection.  The following condition exposed employees to struck-by hazards to the eyes.      A & H Roofing Services, LLC  was previously cited for a violation of this occupational safety and health standard its equivalent standard 29 CFR 1926.102(a)(1) while performing work under the owner's name, Abel Hernandez, which was contained in OSHA inspection number 1458624, citation number 1, item number 3 and was affirmed as a final order on March 31, 2020, with respect to a workplace located at 6575 Greenway Loop, Dublin, OH 43016.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) A & H Roofing Services, LLC located at 467 Landings Loop, Westerville, OH 43082: On and preceding March 23, 2021, the employer did not ensure that the portable ladder used to gain access to the roof extended at least three feet beyond the upper landing that they were using for access.  An extension ladder that was fully extended was used to gain access to the roof.  The section of roof was approximately 18 feet high with the ladder positioned 15 inches beyond the eave of the roof.  The following condition exposed employees to falls up to 18 feet.  A & H Roofing Services, LLC  was previously cited for a violation of this occupational safety and health standard its equivalent standard 29 CFR 1926.1053(b)(1), while performing work under the owner's name, Abel Hernandez, which was contained in OSHA inspection number 1458624, citation number 1, item number 5 and was affirmed as a final order on March 31, 2020, with respect to a workplace located at 6575 Greenway Loop, Dublin, OH 43016.
