Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. Location: On the Flat Roof above the Store Front Employees were exposed to falls from heights up to 12 feet to the ground while conducting roofing activities on a low pitch roof where fall protection was not utilized
