Worksite at 557 Broad Street Meriden CT: On or about March 22nd, 2021, the employees installing metal roofing on a low-slope roof with a minimum height from the ground of approximately 11 feet and 7 inches did not utilize any fall protection system when performing work near an unprotected edge.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. Worksite at 557 Broad Street Meriden CT: On or about March 22nd, 2021, the employees using a 16 foot extension ladder to gain access to an upper landing surface did not ensure the ladder side rails extended at least three feet above the upper landing surface. The ladder side rails extended approximately 12 inches above the landing surface.
