On offhand grinding machines, work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. A) A bench grinder in the maintenance shop had its work rest 1/4 inch from the surface of the wheel.

The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. A) In the maintenance shop, there was plastic container with a colored liquid in it, that was not labeled as to its contents.
