29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about March 22, 2021, located on the west side of a commercial structure, employees were installing shingles while not utilizing a fall protection system. Potential fall heights were over 15 feet.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about March 22, 2021, located on the west side of a steep commercial roof, employees were exposed to a fall hazard while performing roofing work while not utilizing a fall protection system and had not been trained by the employer to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Potential fall height was greater than 15 feet above the ground below.
