29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: a. 532 W. Second St., Plainfield, NJ - Building D - Entry canopy The employer did not ensure that damage ladders were taken out of service. An employee engaged in taping activities for the building exterior worked from the entry canopy on a multi-story residential structure using a damaged Werner, 8-foot, A-frame ladder, missing rear side rails and spreaders, as a means to work. Violation observed on or about March 30, 2021

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a. 532 W. Second St., Plainfield, NJ - Building D Employees engaged in the residential construction activities were not protected from falls. An employee engaged in taping activities for the building exterior worked from the entry canopy on a multi-story residential structure and was not attached with a lanyard and/or wearing fall harnesses or had any other means of fall protection. Violation observed on or about March 30, 2021 Fast Home Service, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1394424, citation number 1, item number 1, and was affirmed as a final order on July 29, 2019 with respect to a workplace located at Building 600, Freedom Village At West Windsorold, Bear Brook Road, West Windsor, NJ 08550
