29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a. 532 W. Second St., Plainfield, NJ - Building F The employer did not ensure ladders were extended at least 3 feet above the upper landing area. Employees engaged in residential framing activities, that included layout of the second level, used a Werner, 24-foot extension ladder for access to the second level that was not extended 3 feet above the landing level. Violation observed on or about April 9, 2021

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a. 532 W. Second St., Plainfield, NJ - Building F The employer did not ensure employees were protected from falls. Employees engaged in residential framing activities worked at or near the outside edge of multi-level structure and were not attached with a fall harness and lanyard or had any other means of fall protection. Violation observed on or about April 9, 2021 Fast Home Service, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1394424, citation number 1, item number 1, and was affirmed as a final order on July 29, 2019 with respect to a workplace located at Building 600, Freedom Village At West Windsor, Old Bear Brook Road, West Windsor, NJ 08550
