29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about March 29, 2021, an employee was using an electrical drill to drill holes into the top plate of a floor frame without wearing appropriate eye protection, exposing the employee to the hazard of being struck by flying debris or particles to the eye(s).

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  On or about March 29, 2021, a plumber was drilling holes to install pipes on the top plate of a floor frame without fall protection, exposing the plumber to a fall hazard of approximately 8 feet to the level below.
