29 CFR  1910.22(a)(3): The employer did not ensure walking-working surfaces were maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.   a) Plating Department- On March 30, 2021, the employer did not ensure that the floor near the chemical storage area, next to the emergency exit, was maintained free of damage caused by  corrosion.   b) Plating Department- On March 30, 2021, the employer did not ensure that the floor near the water treatment area, next to the operator's workstation located near the wall, was maintained free of slip hazards from water accumulation caused by leaks. 29 CFR 1910.28(b)(1)(i)(A):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.  a) Warehouse- On or about March 29, 2021, the employer did not ensure that the unprotected side of the mezzanine storage space, approximately eight feet above ground level, was guarded with a standard guardrail system when employees accessed the area for material or equipment. 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  The employer did not ensure that all flexible cords were inspected prior to use in order to identify hazards and ensure that they were free of defects and damage that may expose employees to injury.  a) Electroless Department- On March 30, 2021, the outer jacket of the power cord on the portable fan was separated from the motor plate housing, exposing live electrical wires.  b) Plating Department- On March 30, 2021, the outlet box on the Plating Line separated from the fixed conduit, exposing live electrical wires.
