29 CFR 1904.7(b)(5):  The employer did not enter a check mark in the box for cases where the employee received medical treatment but was not transferred or restricted:  A.)  Legacy Roofing Services, LLC, Boscov's Roof:  An employee received treatment beyond first aid that included, but was not limited to, stitches.  The employer did not mark the radio button for column J on the OSHA Form 300 for CY 2020 indicating that that injury was an other recordable case.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Legacy Roofing Services, LLC, Bloom Carroll Elementary Building:  The date for an injury in October 2020 was not completed in Column D on the OSHA Form 300 for CY 2020.  B.)  Legacy Roofing Services, LLC, Boscov's Roof:  The date for an injury in October 2020 was not completed in Column D on the OSHA Form 300 for CY 2020.  C.)  Legacy Roofing Services, LLC, Bloom Carroll Elementary Building:  Column F on the OSHA Form 300 for CY 2020 included the injury or illness and the parts of the body affected, but did not provide the object/substance that directly injured or made the person ill.  D.)  Legacy Roofing Services, LLC, Boscov's Roof:  Column F on the OSHA Form 300 for CY 2020 included the injury or illness and the parts of the body affected, but did not provide the object/substance that directly injured or made the person ill. 29 CFR 1926.502(f)(2)(ii):  The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface:  A.)  Legacy Roofing Services, LLC, 2424 West 23rd Street, Erie, PA 16506, east side of the structure, on or about March 29, 2021 and times prior thereto:  Employee were engaged in roofing activities including, but not limited to removing roofing materials.  The warning line system was sagging to within 30-inches of the roof surface.  Employees were exposed to a fall hazard of 14.5-feet.  NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
