29 CFR 1926.416(b)(2): Working spaces, walkways, and similar locations were not kept clear of cords which created a hazard to employees: (a) On or about March 31, 2021, on the fourth floor of the building directly below the roof access hatch, the employer did not insure that an electrical extension cord was kept clear of the walkway or the working space behind it . This exposed employees in and around this area to a trip hazard.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the hazard. (a) On or about March 31, 2021, on the fourth floor of the building directly below the roof access hatch, employees were not protected from stepping thru the partially covered five (5) foot by eight (8) foot floor access hole. This exposed employees to a potential fall hazard of 15 feet to 45 feet.
