29 CFR 1917.50(c)(1): Each crane and derrick was not tested as a unit quadrennially and examined annually. a) Plant Dock - On or about April 1, 2021, employees unloaded fish product from docked vessels with the use of pedestal crane #1 (model: 30 KC 4000, and serial number: 592-0705-90). Pedestal crane #1 had not been tested as a unit quadrennially with the previous test performed on April 14, 2016 and became expired on April 20, 2020. The pedestal crane #1 had not been examined annually, with the previous examination performed on April 25, 2019 and expired on April 25, 2020. These conditions exposed employees to struck-by hazards. b) Plant Dock - On or about April 1, 2021, employees unloaded snow crab from F/V Tempo Sea with the use of pedestal crane #3 (model: 30 KC 4000, SN: 594-0705-90). Pedestal crane #3 had not been tested as a unit quadrennially, with the previous test performed on April 14, 2016 and became expired on April 14, 2020. The pedestal crane #3 had not been examined annually, with the previous examination performed on April 25, 2019 and became expired on April 25, 2020. These conditions exposed employees to struck-by hazards. c) Plant Dock - On or about April 1, 2021, employees unloaded snow crab from F/V Tempo Sea with the use of pedestal crane #4 (model: 30 KC 4000, S/N: 595-0705-90). Pedestal crane #4 had not been tested as a unit quadrennially, with the previous test performed on April 14, 2016 and became expired on April 14, 2020. The pedestal crane #4 had not been examined annually, with the previous examination performed on April 25, 2019 and became expired on April 25, 2020. These conditions exposed employees to struck-by hazards. d) Plant Dock - On or about April 1, 2021, employees unloaded fish products from docked vessels with the use of pedestal crane #6 (model: 50 KTSC 10000, S/N: 97024-022098). Pedestal crane #6 had not been examined at least annually, with the previous examination performed on April 25, 2019 and became expired on April 25, 2020, exposing employees to struck-by hazards.

29 CFR 1917.111(a):The structural integrity of docks, wharves, piers, terminals and/or working surfaces was not maintained.: a) Plant Dock - On or about April 1, 2021, employees performed offloading operations for F/V Tempo Sea, the concrete dock was damaged and were exposed to tripping hazards when walking in the area. Employees operating powered industrial vehicles over the damaged areas were exposed to roll-over hazards.
