29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing operations.  This was most recently observed on March 24, 2021 at 6820 N 92nd St, Omaha, Nebraska when three workers were on the roof of a residential structure and they were not using fall protection.  Angel Rodriguez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1454881, citation number 2, item number 1, and was affirmed as a final order on March 10, 2020 with respect to a workplace located at 10036 Fieldcrest Dr., Omaha, NE 68114.   Angel Rodriguez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1357171, citation number 1, item number 1, and was affirmed as a final order on February 27, 2019 with respect to a workplace located at 2509 S. 186th Cir, Omaha, NE 68130.

29 CFR  1926.1053(b)(10):The tops of a non-self-supporting ladder were not placed with the two rails supported equally:  The employer is failing to protect employees from fall hazards while using ladders during residential roofing activities.  This was most recently observed on March 24, 2021, at 6820 N 92nd St, Omaha, Nebraska when workers were using a ladder with upper sides rails leaning against the pitched side of a residential roof.  The side rails were not supported equally making the ladder unstable which exposed the workers to a fall hazard.
