29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  2411 Rockford Avenue, Rock Hill, MO: The employer exposed employees to eye injuries where the employees used nail guns without protective safety glasses while installing shingles on a roof.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:  2411 Rockford Avenue: Employer exposed employees to hazardous conditions where employees were permitted to work on roofs higher than 6 feet from ground levels to the eaves of roofs without using any fall protection systems.  The fall distance from the eaves of the roof of the front entrance of the house to the ground level was measured at approximately 10 feet.  The fall distance from the eaves of the roof of a second structure in the back of the house was measured at approximately 7 feet 9 inches to the ground level.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    2411 Rockford Avenue, Rock Hill, MO 63119: Employer exposed employees to hazardous conditions where the employees were permitted to access a roof higher than 6 feet using a ladder which did not have the side rails extended minimum 3 feet above the roof.    On 03/24/2021 employees and their site manager used an aluminum ladder to access a roof which was  higher than 7 feet from ground level without extending the side rails of the ladder 3 feet or greater above the eaves  of the roof.
