29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On April 6, 2021 and times prior thereto, employees operated the Hyster forklift, S/N N177V03183M with an attachment used to tow trailers. The attachment has not been approved by the manufacture or evaluated to see how it affects the capacity and safe operation of the forklift.
