(a)  The employer did not develop and document energy control procedures when performing maintenance and servicing activities on equipment such as but not limited to: Chicago Hydraulic Press Brake, Can Crusher, Ellis Horizontal Bandsaw, Rigby Wire & Cable Stripper, and Cincinnati Mechanical Shear.  (b)  The employer did not develop and document die setting procedures for the hydraulic press brake which provided exclusive control of the hazardous motion exposing employees to crushing hazards.
