29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access: a) East side of building, north end, on or about 4-9-21: One employee was descending from an upper scaffold platform while using the frames and braces, exposing the employee to a fall hazard of up to 13 feet 6 inches.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a)East side of building, north end, on or about 4-9-21: Two employees were working on a scaffold platform while using a cross brace in lieu of a full guardrail system, in that the cross brace only served as a mid-rail, therefore the top rail was missing, exposing the employees to a fall hazard of 13 feet 6 inches. b)East side of building, north end, on or about 4-9-21: Two employees were working on a scaffold platform while using a plank in lieu of a full guardrail system, in that the plank only served as a top-rail, therefore the mid-rail was missing, exposing the employees to a fall hazard of 13 feet 6 inches.
