Workers were installing sheathing and other framing pieces using pneumatic nail guns powered at 90-120 psi without the use of eye protection. The following condition exposed employees to struck-by hazards to the eyes.

Carpenters were installing sheathing and framing pieces on a roof that varied from 20-27 feet. The employees were not utilizing proper fall protection which exposed them to falls up to 27 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Luis Alfonso Neaves located at 11551 Poppyseed Drive, Plain City, OH 43064: On and preceding April 7, 2021, the employer did not ensure that the portable ladder used to gain access to the roof extended at least three feet beyond the upper landing that they were using for access. An 8 foot A-Frame ladder was used to gain access to an upper landing that was approximately 10 feet. The following condition exposed employees to falls up to 10 feet.
