The employer did not provide training in the care, maintenance, useful life, and disposal of fall protection equipment for employees such as order pickers. A retractable web lanyard with cuts, frayed webbing, illegible label, and corroded snap hook, was still at the site and available for use.

The employer failed to ensure that employees such as order pickers, were inspecting personal fall protection systems before use. A retractable web lanyard with cuts, frayed webbing, illegible label, and corroded snap hook, was still at the site and available for use.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: a) On or about April 9, 2021, at the warehouse, next to the staging & receiving manager's office, 689 North Terminal Road, Fort Buchanan, PR. A 36-inch working space was not provided in front of a circuit breaker panel, to permit ready and safe operation and maintenance. The circuit breaker panel was partially blocked by a hand-washing sink.
