A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees:  On April 14, 2021 at a worksite located at the corner of Bell Street and Marathon Avenue in Neenah, WI 54956; two employees working inside a trench shield were not provided with a safe means of egress from a trench excavation approximately 6 feet deep. A ladder was removed from the excavation and lying on the ground near the excavation. The ladder would have required being put back in for employees inside the trench shield to egress.

A ladder was placed immediately adjacent but outside the trench shield potentially exposing the two employees to cave-in hazards while entering and exiting the trench excavation.
