29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On or about May 3, 2021, and at times prior, the employer did not ensure that the distance between the abrasive wheel periphery and the adjustable tongue guard did not exceed one fourth inch. Employees were exposed to struck-by hazards as follows:      1.  In the shop at 160 Sutton Ln,  a Delta bench grinder S/N 201088066202038AL,  was used with an abrasive wheel where the distance between the tongue guard and the wheel was one and one half Inches.      2.  In the shop at 155 Sutton Ln,  a JET bench grinder S/N 13070407, was used with an abrasive wheel where the distance between the tongue guard and the wheel was three quarters of an Inch. 29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:  a) On or about May 3, 2021, and at times prior, the employer did not ensure that workers adjacent to welding areas were protected from the rays by screens or shields. Employees in both shops were exposed to eye damage hazards while welding metal products, the employer did not have welding screens in use. 29 CFR  1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  a) On or about May 3, 2021, and at times prior, the employer did not ensure that employees who were using  tight-fitting respirators voluntarily were medically able to use such respirators, and that the respirators were cleaned, stored, and maintained so that their use does not present a health hazard to the users.  A worker was observed voluntarily using a  3M half-face respirator model 6503. The respirator was not stored or cleaned properly and the worker was not medically evaluated exposing the worker to health hazards.
