29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a) On or about April 13, 2021, employees working in an approximately nine (9) feet two (2) inch deep trench installing a water line utility,  were not protected from potential falling objects from above by a protective helmet.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.   (a) On or about April 13, 2021,  employees working in an approximately nine (9) feet two (2) inch  deep trench installing a water line utility,  were not wearing eye protection, exposing the employees to hazards from flying and or falling particles from above or from hazards created inside of the trench while using tools and equipment.

(a) On or about April 13, 2021, employees worked in an approximately nine (9) feet two (2) inch deep trench installing a water line utility with out cave-in protection. This exposed employees to struck-by, crushing and engulfment hazards.

(a) On or about April 13, 2021,  employees working in an approximately nine (9) feet two (2) inch deep trench installing a water line utility, were exposed to struck by hazards from falling rocks, dirt and tools  from above that were not contained or separated  by at least 2 feet from the edge of the trench.
