29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavation(s), or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. (a) On or about May 27, 2021, employees working in an approximately ten (10) foot deep trench installing a water line utility to a residential home, were exposed to struck by hazards from falling rocks and dirt from above that were not contained or separated by at least 2 feet from the edge of the trench. Straightline Grading & Excavating, LLC., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1524799, citation number 1 item number 2, and was affirmed as a final order on October 20, 2021, with respect to a workplace located at 7870 Williamsburg Ct., BRISTOL, WI 53104.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) On or about May 27, 2021, employees worked in an approximately ten (10) foot deep trench, installing a water line utility to a residential home with out cave-in protection. This exposed employees to struck-by, crushing and engulfment hazards. Straightline Grading & Excavating, LLC., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1524799 citation number 1 item number 3, and was affirmed as a final order on October 20, 2021, with respect to a workplace located at 7870 Williamsburg Ct., Bristol, WI 53104.
