29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: New residential construction located on (Lot # 209) 245 Breakspear Lane, Fayetteville, GA 30214, Front side of structure: On or about 04/13/2021, the employer exposed an employees to struck-by hazards while cutting 2x6 ceiling joists, in that employees were not provided head protection while engaged in framing activities with framing lumber being cut directly above other workers.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: New residential construction located on (Lot # 209) 245 Breakspear Lane, Fayetteville, GA 30214, Second floor ceiling joists: On or about 04/13/2021, the employer exposed an employee to falls of 22-feet, 3-inches to the lower level, in that a fall protection system was not provided while engaged in framing activities using the 2x6 joists as a working surface. Rebollar Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1470450 , citation number 1, item number 2 and was affirmed as a final order on 06/23/20, with respect to a workplace located at (Lot A505) 205 Central Ave. Fayetteville, GA 30214.
