29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: At jobsite located at 8707 Silverthorne Rd Largo, FL, an employee was operating aerial lift without fall protection. Employee was exposed to a 10 foot fall hazard, on or about April 13, 2021.

29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. (a) At the job site, employees performing tree trimming work with a chain saw were exposed to fall hazards of up to 10 feet-and were not trained to use fall fall protection from the hazards, on or about April 13, 2021.
