29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  Back room storage mezzanine - On or about April 14, 2021, employees were not protected from falling approximately 8 feet to the lower level where there was unprotected sides of the mezzanine.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a.) Repair Shop - On or about March 31, 2021, employees were not protected from the in running nip point on the wire wheel side of the Baldor grinder where the work rest was not utilized to prevent material from being pulled into the wheel, which resulted in the partial amputation of an employee when the chainsaw chain he was cleaning was pulled into the spinning wheel.  b.) Repair Shop - On or about March 31, 2021, employees were not protected from flying sparks and debris on the wire wheel side of the Baldor grinder where the tongue guard was not utilized.
