29 CFR 1910.305(g)(1)(iv)(C):Flexible cords and cables were run through doorways, windows, floors, or similar openings. Employer is failing to protect employees from electrical shock or burns. This was most recently documented on April 14, 2021 at 2400 W Adams St. Lincoln, Nebraska in the maintenance area of the facility. An extension cord was being used to provide power to a heat cart that was located outside of a roll up door. An extension cord was plugged into the electrical outlet within the building and ran under the roll up door causing the door to put pressure on the extension cord. The cord was visibly damaged by the pressure rendering it unserviceable.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: Employer was failing to protect employees from electric shock and burn hazards. This was most recently documented on April 14, 2021 at 2400 W Adams St. in Lincoln, Nebraska. An extension cord insulation jacket had been pulled from the plug allowing strain to be put on the terminal ends of the cord and exposing the conductors. This extension cord was being used to provide power to a 'heat cart'
