On or about April 14, 2021 and at times prior thereto, the employer did not ensure that employees and employees of subcontractors were protected from stepping into or falling through holes in the walking/working surface. This exposed the employees and the employees of subcontractors to a fall greater than six (6) feet to the concrete below: a. On fifth floor, the open hole around duct work. b. On the fifth floor, center SE part of building directly in front of the stairway system.

On or about April 14, 2021 and at times prior thereto, the employer did not cover the hole openings of a ventilation shaft. The employees and the employees of a subcontractor worked in the areas below these holes and were exposed to struck-by hazards.
