29 CFR  1926.352(b): Positive means were not taken to confine the heat, sparks, and slag from welding, cutting, or heating, and/or immovable fire hazards were not protected from them:  At a facility located at 540 Prospect Street in Combined Locks, Wisconsin:   On or about March 25, 2021, The Boldt Company employees were cutting and removing a stainless steel sleeve which was embedded in the concrete floor during an equipment conversion on Pulper #5 without suitably protecting or shutting down overhead ventilation ducts. A hot particle from the cutting work was pulled into the ventilation pipe resulting in a fire within the duct, exposing employees to smoke inhalation.
