29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard: On or about April 13, 2021, and times prior thereto, at Metal Zinc in Humble, TX, the foot pedal mechanism on the Roto-Die FECO hydraulic bender was not protected from unintended operation that would cause an involuntary actuation of the of the hydraulic press brake.
